There are several benefits of medicines reconciliation, including:
- Better communication between healthcare professionals and improved record keeping
- Fewer medicines-related admissions to hospital. Reduces the risk of missed doses, in particular high-risk medicines and Parkinson’s disease medicines
- Reduced waste (of time and medicines) and less duplication of effort
- Reduced medication errors and adverse drug events.
All members of the multidisciplinary team have an interest in patient care and can benefit from medicines reconciliation. See Table 1 below:
