Medicines optimisation involves understanding the person’s experience, using an evidence-based choice of medicines, ensuring that medicines use is as safe as possible, and doing this routinely within your practice.
The key components of medicines optimisation are summarised in the Royal Pharmaceutical Society’s model in Figure 1.
