Social care workers: establish and address social care needs. They implement, monitor and review care packages at regular intervals, and recommend and commission services such as home or day care from the independent sector.

Domiciliary care workers: usually employed by private care provider agencies to offer support with personal care. May be the first people to identify medicines-related issues, but may not realise the need to refer or know who to contact.

Sensory impairment rehabilitation teams: help with visual or hearing impairments.

Local safeguarding adults boards: multidisciplinary agencies who protect vulnerable adults from abuse and neglect.
