GPs are the healthcare gatekeepers for older people in the community. They provide general medical care and refer patients to other clinicians. Since April 2014, everyone over the age of 75 has had a named, accountable GP.

Geriatricians and old age psychiatrists are consultants based in NHS trusts who provide outreach services to older people in the community. Old age psychiatrists care for older people with mental health problems.

GPs with a special interest (GPwSI) can carry out comprehensive geriatric assessments. For instance, determining a frail older person's medical, psychosocial and functional capabilities, establishing their needs and drawing up therapeutic plans.
